In accordance with current Data Protection Legislation information that you provide in relation to your application to be a volunteer will be held for six months if you are unsuccessful. If you are successful your information will be held on our Hospiscare Volunteer Management system and processed for the purpose of your role. From time to time we may send you updates, newsletters and email where appropriate to keep you informed of news, activities and training.
Information on how Hospiscare hold and process your data can be found at https://www.hospiscare.co.uk/about-us/privacy-and-data/
Unfortunately we are unable to accept applications from anyone under the age of 14.
Hospiscare recognises the contribution that ex-offenders can make. A person’s criminal record will not necessarily bar that person from volunteering. Suitable applicants will not be refused a role because of offence(s) which are not relevant to, and do not place them at or make them a risk in, the intended volunteering role. Each disclosure is reviewed on an individual basis and will take relevance, seriousness, circumstances and any other pertinent information into consideration.
It is important for you to understand that failure to disclose all unspent convictions could result in changes to Hospiscare agreement with you to volunteer.
NOT TO BE COMPLETED IF APPLICANT IS UNDER 16
I confirm that the information I have given on this form is correct and complete, and understand that misleading statements may be sufficient for cancelling any agreements made.
I agree that if I am successful in my application I will:
1. Adhere to Hospiscare policies.
2. Apply and uphold Hospiscare values at all times and to all aspects of my role.
I understand that:
1. certain roles may require a Disclosure and Barring Service (DBS) application
2. because of the sensitive nature of such roles they are exempt from the Rehabilitation of Offenders Act 1974 and therefore spent criminal convictions may be disclosed.”
